From the photo, this doesn’t appear to be a bald spot but a natural hair part or crown whorl, which can sometimes give the appearance of thinning. At 23 with a full head of hair, a transplant isn’t typically needed.

The main questions to ask yourself is whether this is new, is it worsening or it has always been there. If it has always been there then there isn't much to do, as it is normal for hair to grow in different directions creating a part. If it is new, or worsening, then the best course of action would be to speak to your dermatologist to see what may be causing it, and what interventions may be in your best interest.

You may want to see your dermatologist. If the hair parts like this just because that’s your normal, there is not much that can be done although your choice of shampoo and general scalp health might make a difference in these cases. If your hair parts like this due to early androgenetic type hair loss (or some other hair issue), there may be strategies to stop the process from getting worse or even thicken it up. I would encourage you to visit your dermatologist and review the area up close.

If there is an isolated area of hair thinning you could apply topical minoxidil as well as inject PRP into the area. Alternatively, you could part your hair in a different manner so that the scalp would not be visible.
