I am a transgender woman who had breast augmentation surgery 4 months ago, in August 2017 (575cc round smooth silicone mod+ profile submuscular). I have Pectus Carinatum. My breasts are spaced far apart without a bra, and when I lie down, they fall into my armpits. I've been told there's nothing more that can be done to bring them closer, because that would make my nipples point outwards more. Could internal sutures fix the lateral displacement? Or other fixes to bring them closer together?

Your breast implants demonstrate lateral displacement when lying down. This is a difficult problem to treat, but switching to textured breast implants and a new pocket can help. You will likely need revision surgery.

Thanks for your question and photos.  It appears that your implant pockets have stretched, or were over dissected laterally, as well as some bottoming out, based on the scar position.  I think that you could get significant improvement with internal suture fixation and revision.  Consult with a surgeon who has experience with revisionary breast augmentation.  Best Regards.
