I have a very large & long lower face. I've read that for trans patients a Lefort 1 can be used to decrease the length of the face and philtrum. Are photos of my goals realistic? Ideally, I hope to end up with aesthetically too short a philtrum so that I can then have a Y to V lip lift (I scar too poorly to get a lip lift that leaves a viable scar). I'd also like to have sagittal osteotomy of the mandible, v-line osteotomy, chin shortening & sliding genioplasty. Can these be done simultaneously

Hello ilovemacaroons, It's not possible to completely predict how much sagging you would have. A useful piece information is the amount of bone that will be removed. More or excessive bone removal would lean things towards more sagging. Your soft tissue is stripped off of the bone in the V line...
You do have a short chin that is round as most short chins are. From a bony standpoint a t-shaped genioplasty is needed to being it forward and narrow it. Another option is the v-shaped anatomic chin implant if one prefers a non-bone moving approach.
