I have a small eyelid assymetry which is genetic since i had it since i was a child : my right eyelid is slightly more hooded than the other. My question is: can i reduce the assymetry with upper blepharoplasty without having to remove a lot of skin and fat? I only want a small change and i am afraid of getting hollow upper eyelids. I am not interested in any botox or lasers.
Answer: This is a tiny issue that will be impossible to get the surgery you need to be happy. Sorry, eyelid surgeons are just not this good. A blepharoplasty to address the "slightly more hooded" right upper eyelid, assuming you can get someone to operate on it, will be a disaster. If you carefully study your photo, you will notice that you carry your left eyebrow higher than the right eyebrow. The right upper eyelid is slightly more ptotic than the left upper eyelid. It suggests that you have some latent ptosis in the left upper eyelid that is stimulating more eyebrow compensation on the left side. I can not tell you precisely what would happen if you have a "conservative" blepharoplasty on the right side without a detailed, in person consultation. What I can tell you is a guarantee that 99.9% of the eyelid plastic surgeons you might see will misunderstand what you are looking for and will make you much less happy with your appearance. Caution is advised.

Answer: Upper blepharoplasty Although an in-person exam would be necessary to provide a definitive answer, based on the photo you have provided it does appear possible to give you a crease that is higher and more symmetric with upper eyelid surgery. During the surgery a small amount of fat will be removed, and your crease will be formed and set at a bit higher. If you want the best idea of what is possible for your eyes, do your due diligence, find an experienced surgeon, and have him/her evaluate you in person and discuss your options. @drdonyoo
