When will I start to see my skin clear on accutane?

Accutane is a go-to medication for patients with recalcitrant cystic acne. It can reduce pore size and also reduce sebum production in your skin, making it less oily; and in turn, dryer. Accutane can be used to control acne, but patients receiving the medication should be carefully monitored. In terms of skincare, always wear an appropriate sunscreen and reduce direct sun exposure. In my patients, I recommend a good ceramide moisturizer such as Elevase whenever they are on Accutane. This will keep the skin hydrated and reduce the oiliness of the skin that is associated with oil-based moisturizers.

The doses seem quite low especially for being in the third month.  I have not had one patient not clear their acne on isotretinoin (Accutane) but the doses have been higher.  Treatment typically takes 4-6 months and I tell my patients not to expect improvement until they have at least completed their second month.  Good luck.Best, Payman Kosari, MD, FAAD

I have over 30 years of experience prescribing Isotretinoin.   Accutane brand in not actually available in the US but other brands are and are of equal efficacy.   Dosing approaches can vary but usually our patients will receive 1 mg/kg of body weight daily for 5- months with great results.   We only offer this to patients with true nodulocystic acne.   90% will clear over this time period while a few will need a longer course even up to 9 months.  All patients are part of the iPledge monitoring program and will need monthly lab tests drawn.   Remember that taking you medication with food increases your blood level, hence, raising your success rate and speeds things along.I hope this is helpful.Steven L.
