Payman Kosari, MD, DABVLM is a board-certified dermatologist practicing at DLVSC in Charlotte, NC and the surrounding areas since 2013. Dr. Kosari grew up on the west coast and remained close to home for undergraduate education, receiving his Bachelor of Science degree in biology from the University of California. Next, he moved on to the midwest, earning his medical degree from the Chicago Medical School. I had struggled for years trying to improve the moderate acne scarring in my chin area. I saw some improvement after several dual fraxel treatments but the lasers always flared up my acne horribly for weeks after and caused more scarring. I really wanted to try subcision and found Dr. Kosari, one of the only physicians who perform this procedure in our area. Dr. Kosari used a combination of subcision and PRP injections with microneedling. I will admit the procedure was kind of scary and I was super nervous about it. Afterwards I looked like a bruised chipmunk for a couple of days from all the PRP in my face. After the swelling and bruising healed I had some microneedling marks on my face for almost 8 weeks after the treatment. These marks were not noticeable to others and easily concealed with a little powder. None of that matters though because within a couple weeks of the treatment almost all of my scarring disappeared. Nothing has ever worked so well for me, I cannot thank Dr. Kosari enough! I am 6 months out and it still looks just as good. Additionally PRP is actually good for acne so I was breakout free for months afterwards. Over the years I spent thousands on laser treatments so I couldn't believe that one procedure from Dr. Kosari was the answer I was looking for. If you have deeper scarring you will need multiple treatments but I would definitely recommend seeking his advice and considering the procedure.
I had Miradry Fresh treatment to my underarms 2 days ago. Dr. Kosari and his assistant Maryanne explained the treatment to me step by step during my consultation. The treatment was exactly what I expected and to my surprise not painful at all. I have some lumpy type swelling and injection bruising which I was told would happen. I have not needed to wear deodorant under my arms because of no sweating. The best of all I was able to resume normal activity after 2 days. I highly recommend this practice to anyone looking to have this treatment.
After suffering with cystic acne in my teens, I was left with acne scars. Acne scars are not just on the surface but affect your self esteem. I've searched for years for any thing that would help. I read about subcision and chose Dr. Kosari even though he is 1.5 hours away. Treatment options were explained to me and we began with subcision at my next visit. I noticed slight improvement. My next appointment we did subcision and PRP. Three weeks out and the results are significant. Friends, who knew nothing of my treatment, have commented on my skin. Absolute miracle-worker! I had a nasty deep scar on my nose that I had been dealing with for two years with at a different practice. Dr. Kosari was able to quickly assess the problem and treatment. He was always extremely professional and honest. Results were far better than expected, and I could not be happier! I have to also give props to his nurse—she is equally wonderful—smart, compassionate, and down to earth. They make an outstanding team!

I had veins running from temple to under my eyes. They were awful, especially my left side. I had two under that eye and one that was swollen and so blue, it was gross! Not many doctors perform this procedure due to it being so close to the eye but I found one and it was awesome! One treatment, gone within minutes. Minor swelling for a couple days. I saw my before pics yesterday and I had forgotten how bad it really was. Amazing results.
After my first initial treatment within days my keloid scars began to flatten out and also began to become lighter in color. I have just received my second treatment and looking forward to seeing how the results turn out. The initial treatment showed promising results well enough that I would highly recommend this treatment, especially from Dr. Kosari.
I have nothing but good things to say about Dr Kosari. He offers a full gamut of acne scar treatments at his practice, so I was assured that I would get treatment catered to my scar type and not 'one size fit all' type treatment. Since I was an out of town patient, Dr Kosari did a comprehensivd phone consultation with me after I sent my pics to him.We decided on a combo treatment of subcision + infini + prp for my rolling and boxcars on temples and cheeks. I flew from Toronto to Charlotte to get this treatment done, so I was his first international patient :) About the procedure: the treatment started by him marking all my scars under harsh angled lighting so he won't miss any a scars during the treatment. He took his time numbing the face, so the whole treatment was pretty much painless. Following that he did subcision using 18G nokor needle on the scars he marked. I could hear the tethers breaking, which was an assurance that this was the right procedure for me. This was followed by 5 passes of infini rf at various depths and energy settings. Finally prp and healite was performed for faster healing and hopefully help collagen remodeling as well. In the end, I was given the appropriate antibiotic and steroid creams to avoid any infection and unnecessary inflammation. Following day, I had a follow up with Dr Kosari. He ensured that the recovery is as expected. And I flew back to Toronto. His staff (Jessica, Tamara)were very nice, informative and entertaining as well. I've had several small, flesh colored bumps on my face that left my skin looking and feeling "textured". Dr. Kosari diagnosed these bumps as sebaceous hyperplasia and recommended a "cosmetic cleanup" session that included electrodessication of the bumps. Both he and his assistant were absolutely wonderful in answering my questions before starting the procedure and in informing me of what was going on during the procedure. I appreciate his patience, reassurance, and skill. At my follow up session 4 weeks later, he offered to clean up three spots that the first session didn't finish at no additional charge. He has one of the best bedside manners I've experienced - professional, yet personable at the same time!
The temporal branch of the facial nerve crosses in this region but as long as the physician remains superficial, there will be no issues at all. Best of luck
Don’t separate these two if you can. They work synergistically and will give you the highest yield. If you have to choose one, I would go with subcision as these scars need to be freed from the underlying skin for any appreciable result. Best of luck
Too early in the life of the scar.scars can continue to remodel over the course of 1 yr. Three weeks is still the very early beginning. I agree with massaging at this point. Scar Away is a silicone based topical I recommend for scars. Vascular lasers can get the red out. Intralesional steroids and resurfacing lasers can flatten the scar. My advice would be to massage and use a silicone based topical though. It’s early enough and that’s all you may need. Best of luck
Why not combine these treatments? I'm sure there was a reason but combining them will save visits and likely be more effective given their synergistic effects. It's hard to give a full assessment without knowing more details though. Best of luck
I recommend two weeks. With that said, plenty of patients have flown out at the 1 weeks mark. I am usually ok with this but like them to ambulate frequently while on the plane to be sure to wear their compression stockings. Best of luck.
