Rhinoplasty or alarplasty for my Asian nose? (Photo)

Question

Hello My concerns about my nose are the width/unevenness of my nostrils and the flat shape from a tilted angle. I was wondering if these could be addressed with just alarplasty alone or should I undergo a full rhinoplasty? I am happy with my profile and I don't wish to have a different nose shape - it's just too wide now, especially when I smile. However if I do go under the knife, I do want it to look natural and not like I just pinched my nose. Please advise! Thank you very much

Dear Onespecialplace,Thank you for your photos and questions.  In my opinion, you will need modification of both the alar base and the tip of the nose to achieve your desired goals.  An alarplasty will narrow your nostrils and the width of the lower third of your nose.  However, an alarplasty will not change the shape of the tip of the nose.  I do feel that the bridge and especially the upper portion of the bridge, called the radix, needs to be slightly higher if the tip is refined.  Ideally you would have a full rhinoplasty.  You could always get just the alarplasty first, usually done under local anesthesia, to see if you would be satisfied with this smaller procedure alone.Aloha,Shim Ching, MD
