Can a lower blepharoplasty result in a negative canthal tilt?

I like to first examine the tone of the lower eyelid before surgery. If the tone is poor the patient is at increases risk for developing ectropion. (I.e. the lower eye lid pulls down, pulling the lower eyelid away from the eyeball. If the patient has good lower eyelid tone this can also happen if too much skin is removed from the lower eyelid. If one wants to change the can’t or angle of the lower eyelid either a canthopexy or canthoplasty need to be performed. these are two different techniques for changing the angle of the lower eyelid tendon. Many lower blepharoplasties also include either a canthopexy or canthoplasty. These fix the lateral cants to the orbital rim. If this fixation is misplaced, the cants will be either too high or too low. This can be corrected, but you need an expert in eyelid surgery to do this. Avoidance is also by choosing a surgeon who is an expert in eyelid surgery.
