The best implants are silicone gel placed retro-pectoral since they look and feel more natural, are less likely to ripple and have maximum mechanical support.  However, large implants may not fit and if you attempt large implants, they will extrude inferiorly and laterally. You will have to decide between large implants, which will require multiple revisions or smaller implants that will be more stable.  Aligning the areola, breast tissue and implant over the bony prominence of the chest wall gives maximum anterior projection with a minimal size implant.

The capsule forms around a breast implant serves as an internal bra. Nevertheless with very large breasts it's best to protect them with the bra particularly during athletic activity.

The bigger the implants, the more all potential problems can be exaggerated: existing asymmetries are more evident, droop, stretching of the skin/breast tissue.  Certainly they can be downsized and the breasts lifted as necessary in the future, but consider your goals and think about the future before you jump into this.  I will just assume you are working with a board certified plastic surgeon because if you are not you should.  Best of luck to you.

In general, yes.  I tell all of my breast augmentation patients, especially younger ones such as yourself, that they will almost certainly want revision surgery in the future especially if they are having large implants.  Breasts change during pregnancy/breastfeeding as well, and those changes can also lead to the desire for revision surgery.

You can expect undesired stretching of your skin to some degree and could need a lift down the road as well as downsizing the implant, depending on what you are trying to correct.  If that size will truly make a difference in your life, then go for it.  But if you're just wanting to be larger, consider something that will fit on your frame better.  Wish you the best!

Even small breasts sag, the bigger you are the saggier you can get. There is no way to prevent sagging.  Good luck.

Unfortunately there is no way to say for sure whether the implants will sag with time. But the larger the implant, the heavier it is, and the more it will stretch the skin of the breast. So if you are young, and your breasts go through changes over the years (after having children, fluctuate body weight, etc.) there is a good chance you may need a lift down the road. Even if your body doesn't  go through any of the above mentioned physical changes, there is still a chance the a large heavy implant like the one you have pictured may cause you to need another operation down the road. We can never say never for future events. So you should get implants and live your life. If you sag in the future than address that issue at that time. 

All tissues eventually sag due to the pull of gravity on our bodies and because surface skin and internal structures degrade during the normal aging process.  The more weight is added in, the faster and more dramatic the sagging effect is.  As some of the other surgeons have suggested, with large to very large implants, it is quite likely that you will want to have your breasts lifted in the future.

If you're working with a board certified plastic surgeon who performs many breast enhancement procedures annually (meaning hundreds), he or she should be educating you on the pros and cons of various size options.  Work with your surgeon and choose implants carefully--with all measurements (base width, projection and amount of fill) that fit your frame well.  Even though you are a fan of the extra large look, you probably don't want your breasts to appear strange by spilling over the sides of your chest, for instance.

In general, yes - the larger the implants, the greater the effects of gravity and volume expansion on the elastic forces in your breast and skin.

Be mindful that you ideally want to avoid implants too large for your body proportions as it will look unnatural and age poorly with you. Revisional surgery for implants that are too large is also more complicated as it is harder to achieve symmetry, healing and good scars.

Your breasts currently have good shape and form but lack volume. Your body is also slight. So be mindful not to overdo the volume on your breast implants - it is hard to come back from a breast envelope that has been stretched too much by an overly large implant.
