I have small tight perky breasts. I'm looking to get very large implants. I'm wondering if it would eventually sag and if I will need a breast lift sometime in the future due to the very large implants?

Thank you for the question and pictures. Using breast implants that are too large for your frame can be problematic in many ways. That does not mean you cannot achieve a “very large” result but care must be taken not to cause problems by using implants too large for your anatomic dimensions. Whether or not you will require breast lifting in the future will be dependent on many factors such as changes in weight, changes with pregnancy, and/or whether you choose to downsize breast implants in the future. Skin elasticity and genetics may play a role in the longer term.

Answer: Avoid saggy large implants
The best implants are silicone gel placed retro-pectoral since they look and feel more natural, are less likely to ripple and have maximum mechanical support. However, large implants may not fit and if you attempt large implants, they will extrude inferiorly and laterally. You will have to decide between large implants, which will require multiple revisions or smaller implants that will be more stable. Aligning the areola, breast tissue and implant over the bony prominence of the chest wall gives maximum anterior projection with a minimal size implant.

Answer: Large breasts as a result of breast implants are less likely to sag.
The capsule forms around a breast implant serves as an internal bra. Nevertheless with very large breasts it's best to protect them with the bra particularly during athletic activity.

Answer: Large breast augmentation
The bigger the implants, the more all potential problems can be exaggerated: existing asymmetries are more evident, droop, stretching of the skin/breast tissue. Certainly they can be downsized and the breasts lifted as necessary in the future, but consider your goals and think about the future before you jump into this. I will just assume you are working with a board certified plastic surgeon because if you are not you should. Best of luck to you.
