After unsuccessful midface filler for level 1 nasolabial folds I believe the major issue is actually lack of bone support. Orthodontist confirmed small jaw as teen. Other signs: Deep bite, recessed chin, fat pocket under chin though underweight, lip incompetence, down turned corners of mouth, perioral mounds, very noticeable worsening of skin folds (nasolabial and melomental) and sagging when looking down but not when facing straight. I believe this is due to a small or recessed jaw? TMJ maybe.

The answer depends in part on what you perceive as the cosmetic issue. You have a vertical facial growth pattern which typically leads to a more narrow ramus/gonion, and a ,'longer face'. Alterations to the mandible are common including reductions and augmentation
You appear to have a lower jaw which developed asymmetrically. Xrays would demonstrate the problem to you. consult a Maxillofacial surgeon in your area. The management is well described.
