Can you advise on clicking in columella 7 months post-op?

We utilize columellar shoring strut grafts in nearly everyone who undergoes rhinoplasty, and sometimes the graft clicks on the bone on the floor of your nose. When this is bothersome, the length of the C-strut may be trimmed in the office with no downtime. We have performed this through an incision in the mouth, under the upper lip. Hope this helps. The clicking you hear is most likely a graft that is sandwiched between the native cartilages of your columella (the medial crura of the lower lateral cartilages) moving side to side against the lower edge of your septal cartilage. This is normal for it to move relative to the septum, as it is fixated with the normally mobile native cartilages. Over time, that will become less apparent and it will not lead to a negative outcome.

First thing that comes to my mind, is columella cartilage graft, frequently placed in front of distal septum to maintain good projection of nose tip. It is very frequently used in contemporary rhinoplasty. Other options are distal extension graft (similar) or mobile distal septum, dislocated from nasal spine. Only your surgeon, who did surgery may have better explanation. However, if that does not affect appearance of the nose and nose tip, it should not bother your too much. Good luck.
