Based on your description and photograph, it is likely that you will do well with breast reduction surgery; this operation, is one of the most patient pleasing operations we perform. Most patients undergoing breast reduction surgery wish to achieve enough of a reduction to help with their symptoms while remaining proportionate with the remainder of their torso. With the goal of improving communication with my patients (determining their goals in regards to breast size after breast reduction surgery) I find the use of photographs of “goal” pictures (and breasts that are too big or too small) very helpful. I have found that the use of words such as “natural” or “C cup” or " proportionate” etc. mean different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate.Key to success: select your plastic surgeon carefully and communicate your goals carefully as well. In general, going from a very large size to a much smaller size and still maintain a nice shape can be difficult. Realistically in my opinion, large "C" small "D" is probably more reasonable. Best to be seen in person.
For most women, a breast reduction is a safe procedure, and gives many relief from neck and shoulder pain. I have taken G's down to D's and C's. If you have any medical issues, you may need your physician to give clearance for surgery. I would suggest meeting with a plastic surgeon and discussing all your concerns.

Based on your photograph, I see no reason why you wouldn't be an excellent candidate for a breast reduction procedure, which would also place your breasts into a more youthful position. And, with current techniques, you are may also be a candidate for a short scar techniques which would improve your shape and reduce scarring.
