My breast are very heavy size 38M/N at this time. I would like to reduce to about 38D but do not want to get a FNG. Is this possible? I am in Michigan.

If your measurement from the sternum to the nipple is around 40 more indicative of a need for a free nipple graft. Consult with several board certified plastic surgeons for an examination and treatment options.

Answer: Do I have to have my nipple removed during a breast reduction? I think you would do fine with a standard breast reduction technique that leaves the nipple and areola on the breast the whole time--no free nipple graft. Be sure to see a plastic surgeon who does a lot of large breast reductions. Best wishes!

Answer: Reduction without free nipple graft (FNG). You can have a reduction without a free nipple graft, and for those unaware, the FNG removes the nipple completely and reapplies the tissue as a graft after the breast is reduced and reshaped. The nipple will lose sensation and a bit of colour but will will allow a substantial reduction. With a proper goal and expectation you can have a reduction with a T pattern and be close the D cup, the approach we prefer.

Answer: Free nipple graft reductions FNG reductions are rare but are good in specific instances. I can't say without an exam but I'd doubt you could get anywhere near down to a D without FNG surgery. The problem is that when your nipple is so low, you have to keep a great deal of breast tissue to try to keep enough blood flow to help the nipples survive. A FNG isn't held back by this requirement because the nipples are applied at the end as skin grafts.
