When belly buttons are too small it is much easier to make them a more natural size than it is to make large belly buttons smaller. This procedure can sometimes be performed under local in the office. Find a plastic surgeon whose belly buttons you like and see them for a consultation. This is something that can be done typically under local anaesthetic in an office setting. My suggestion would be to contact your plastic surgeon and find out what your options are. All the best. 

Yes. an umbilical incision can be revised and the opening enlarged. Obviously you still are at risk for recurrent scarring. I would speak to your plastic surgeon or consult  another, preferably certified by the American Board of of Plastic Surgery.
