How can I solve my neck / double chin problem? (Photos)

Question

I'm 32 and I'm struggling to solve my double chin problem. I'm very thin and always have been. I'm pleased with the appearance of my face (from the front) but I've always been upset with my profile—my face is quite thin except for under my chin. I had submandibular liposuction which has given me very little results. Do I have deep-lying fat or is it my skin? I added images of me pulling back/hiding the chin to show what I'd ideally like to achieve if it is possible.

With significant neck sagging you would need a facelift / neck lift with muscle tightening (platysmaplasty). Thank you for your question. If you feel your skin hasn’t been as responsive to liposuction as you’d like, you may consider the more long-term benefits of a neck lift. Unlike a facelift, neck lifts target the neck skin directly to provide a tightening, jaw-defining elevation. Based on your photos, it doesn't seem that you have enough skin looseness to warrant a standard necklift, especially since that would leave visible scars behind your ears.  Therefore, I suggest  liposuction of the front of your neck through a short horizontal incision under the chin, possible removal of any deep fat behind the platysma muscles under the chin, and tightening of the platysma muscles under the chin, all through the same incision.  The only other option, I can think of (if needed) is to place a long suspension suture through the same incision, running just under the jaw, along both sides of the neck, tightening the muscles slightly backward toward where each suture is tied in the groove behind each ear.

It is very likely, based on these photos and short of in person evaluation, that combination of liposuction of neck and chin augmentation with implant, would improve your neck contour. Dear djc8637958,Thank you for your question and photos. You are a good candidate for a deep necklift. A deep necklift addresses the platysma muscle, deep neck fat and deep neck musculature. Your neck shape is genetic. It can be optimized. It will be scarred because of the liposuction you've had. The limiting factor of how sharp your neck shape is your hyoid bone. I would also consider a small chin implant to lengthen your chin. I recommend you consult with a board certified facial plastic surgeon. I hope this helps!

The photographs demonstrate a recessive chin profile, therefore consider placement of a chin implant. the fat deposits in your neck are most likely located both above and below the platysma muscle in the neck. A surgical neck lift procedure can accomplish removal of both compartments of fat located above and below the muscle in addition to a platysma-plasty to significantly improve the jawline.
