Best way to treat the jowls? Prefer no surgery but any other options? I didnt really notice them until the last 4 years then my face seemed to age quickly?

Photos are no substitute for an in-person examination that allows for manipulation and palpation of the tissues. However, from what I can see, your problem is the kind I see several times per day practically every day in my practice.  You would likely obtain significant improvement in the jowl formation along your jawline with the skillful use of volumizing fillers. The injection of a volumizing filler, such as Voluma XC, which I would choose in my NY office, in the cheekbone regions can lift and contour the cheeks and diminish both the smile lines below and to some extent, secondarily the jowl formation along the jawline. The use of Voluma XC in the region of in front of the ears and particularly along the jawline would likely lead to substantial straightening and definition and finally, if the chin, which seems somewhat recessed in the photos, were augmented with a volumizing filler, this would additionally not only sharpen the jawline, but improve the profile view, as well. The results typically yield an immediate "Wow!" reaction.  In my Israel satellite facility, where a far greater number of regulatory agency approved volumizing fillers are available, I would choose Stylage XXL and Aliaxin EV for the cheeks, Restylane Volyme for the jawline, and Neauvia Intense Flux for the chin.  You would be wise to seek consultation and treatment by a board certified aesthetic physician and make sure to ask to see his/her personal before and after photos before agreeing to proceed. For most patients, facial cosmetic surgeries can be done in the office under local anesthesia - mini facelift, mid face lift, neck lift, blepharoplasties etc. For example, bilateral upper & lower blepharoplasty, mid face lift, Short Scar facelift and neck lift usually takes me about 90 minutes in the office under local anesthesia. Thank you for your question.  The only way to remove loose skin around the jawline would be through a facelift.  This doesn't need to be really invasive and can be performed a under local anaesthetic.  The facelift I perform is under a local anaesthetic which means a significantly reduced downtime.
Most of my patients want a facelift however do not want the downtime associated.  The Concept Facelift and Necklift is the procedure I perform, which combines the latest surgical techniques with the use of local anaesthetic to dramatically reduce the risks associated with traditional facelifts, while significantly reducing the bruising and downtime of your recovery, from months to a matter of days.

I would say the best method to reduce or improve jowls would be with a facelift.  There are some non-surgical treatments that can improve jowls, but not to the same degree that surgery could.  Fillers can sometimes be used to help as well as radio frequency treatments such as FaceTite.

Jowls have different causes in different people. If there is only a bit of excess fat with tight skin, liposuction alone will work well. For other people, use of a filler like Voluma that lifts the cheek tissue is of great help. In your particular case, however, you have a combination of loose skin and excess tissue. A minimal surgical lift of the jawline region done under local anesthesia may be quite effective. See a plastic surgeon for an evaluation.

Based on your photo, you are a great candidate for a facelift which would provide you with the most improvement in your jowls. Other minimally invasive options are filler to the pre-jowl sulcus and marionette lines or injectable radio frequency to help tighten the area. These can be considered if surgery is out of the question, though the results will not be as significant. Best of luck!
