250cc, how much can a round, submuscular implant shift? (Photos)

Question

I had 250cc round implants placed in the sub-muscular pocket. My left side looks and feels great, but my right side looks weird. Like it has sunken or flipped halfways, which creates this pointy shape. If I press in and upwards the shape looks more round for a few seconds, but then it drops again. Also, I can feel this implant a lot more, so I'm wondering what could have happened? My surgeon refuses to form a proper opinion, so I'm hoping to get an honest one here.

Allow me to answer your question as best i can based upon your photos. However, please keep in mind that an in-person, or at least a virtual, consultation with physical exam is first necessary before any legitimate evaluation or final surgical recommendations can be made.

That being said, indeed you do have asymmetry with your right side looking quite droopy. This most typically represents an implant that is NOT intramuscular (or “submuscular”) but rather on top of the muscle. Another possible explanation would be an implant rupture, if your implants are silicone gel instead of semi-soft silicon rubber, like those only available in the USA. An MRI scan would help further elucidate what is going on here. It’s not unheard of for inexperienced surgeons to claim that the buttock implants  are completely within the muscular when in fact the implants or not. When implants are properly placed within the gluteus muscles, they do not and cannot spontaneously dislocate to a position on top of the muscle. Hope this helps.

You are an excellent candidate for a simultaneous butt implant removal and fat grafting transfer. This procedure is technical and requires advanced tools and expert technique. First, an ultrasound probe must be used to ensure that the transferred fat does not get into the pocket created by the implant once the implant is removed as this will result in the death of all the fat. Second, use of VASER liposuction to harvest the fat will be required since you probably don’t have much fat to begin with since you elected to have a butt implant rather than a BBL the first time go around. Additionally, VASER allows for thinner layer of fat grafting that is required for refilling of the area where the implant was previously positioned. This is because this layer has been thinned out and can only accept liquified fat that is harvested using VASER technology. Fat should also be harvested with 1) a closed looped system to process the fat in order to optimize transfer time and avoid your fat from becoming contaminated and 2) strategic micro-packet transplant technique that maximizes graft take. These processes have demonstrated over 90% fat graft take. Finally, we must consider whether you will need a skin tightening maneuver such as a lateral thigh and buttock tuck that might be required if you have sagging buttock skin. Last but not least, attention must be given to providing you the buttock shape that YOU desire. Implantat  placeras vanligtvis inte i ett submuskulärt plan. bröstimplantat är men det finns inte utrymme under gluteusmuskeln för ett implantat. gluteala implantat bör placeras intramuskulärt.

Om det verkligen är så implantatet placerades så gled det ur fickan och är nu subkutant.

Gluteala implantat har en lång lista av oönskade biverkningar och hög grad av komplikationer.

Du har förstahandserfarenhet av varför de flesta plastikkirurger inte utför denna procedur.

Om du vill behålla dina implantat behöver du en revision.

I slutändan misslyckas många människor helt enkelt gluteal augmentation med implantat eftersom proceduren inte gjordes korrekt första gången.

Även när det görs av dem som har tillräcklig erfarenhet har proceduren fortfarande en hög komplikationsfrekvens.

I händerna på oerfarna kirurger är resultaten dåliga för det mesta.
