I would first use Morpheus micro-needling and Renuvion to tighten the skin as much as possible, then later do very careful fat transfers to fill in the dips.

Hi and welcome to our forum!From your photos, I note significant skin surface irregularity and surface scarring of the upper arms, most likely involving the underlying soft tissues as well. In addition, the skin envelope of the upper arm appears lax as well. The risk / benefit ratio must be considered. Surgery in a scarred area where the blood supply to the tissues has been previously manipulated incurs increased risk of complication. Tightening the skin by removing the excess adds another significant scar extending from armpit to elbow on the inner aspect of the arm. This may improve but not eliminate the skin contour irregularity. Visit a board certified plastic surgeon for in-person evaluation.Best wishes...
