I have a tiny flat red spot that I just noticed on my left breast, my breast augmentation was transaxillary 405cc, 1 year ago. Should I be concerned? It doesn't feel sensitive or painful.

Yes, I believe a lift to raise the nipple and breast up is essential, as you appear to already have grade II ptosis. Since you are a full C, a 400 cc implant will certainly make you "full". You will have more than enough volume with that measurement.
