I had the all on four done in June 2014. Immediately after surgery I swelled more than normal due to an allergy to the agents in the temp. The temp not only caused me to have serious allergic reaction, My midline was off and the implant sat too far forward making me look like bucky the beaver, I have been on prednisone since the surgery and have gained weight .I am concerned about the long term effects of the steroid. I cant smile due implant being too far forward restricting my natural smile.
Answer: I went to another dentist for a second opinion and was told the implants were placed too far forward. Any suggestion? The cosmetics of your case can usually be improved by changing the prosthesis on the implants. Implants may not be solid if not in the bone but if they are integrated it shold be possible to create esthetic restoration.I am concerned about long term prednisone therapy. Do not assume your allergic reaction was from the temporary material it could be anaesthetic or other gents.Consider doing Clifford Test or visiting an allergist

Answer: Implant position for the All on 4 procedure Implant placement position is important to achieve proper aesthetics and phonetics with the All on 4 procedure. If the placement of your current implants is not allowing you to achieve an aesthetic result, then you may need to have additional implants placed. Placing additional implants will allow the two implants that are too far forward to be "put to sleep".In essence, that means they will not be used to support your prosthesis and will simply be disengaged. Answer: All on 4 treatment Hi, it is difficult to asess from the image if position of the implants is the issue or the attachment to the implant.The part that goes over top of the implant can be redirected into multiple angles as needed to correct the situation but if the implant is too far forward and does not have a thick area of gum support around it , then over time it can fail due to the tension pull from the muscles in the area.This would need to be evaluated in person.

Answer: You can improve with a new prosthesis The implants can't change the position. But you can improve with a new prosthesis. Don't worry, the middle line can be change the only problem will be the metal maybe show little bit, but a lot less than now. I don't now why do you continue with prednisone .. If you are in pain .. Something is wrong. If you don't have pain ask about stop the medication.

Answer: All-on-Four incorrect implant placement I am sorry to hear of your poor experience thus far. The good news is that:1) You have already waited 9 months. That way past the necessary amount of healing time. You can finally get rid of that temporary and begin the process of getting a permanent implant denture made. 2) There are several techniques that a dentist can use to compensate for less-than-ideal implant placement. There are several different parts and pieces to use and even different types of materials that the implant-denture can be made of. If you are having doubts, I would strongly recommend that you see a Prosthodontist for a consultation. A Prosthodontist is a dentist that has had 3 additional years mainly dealing with these complex rehabilitations.3) During the fabrication of the permanent implant denture, there is a try-in stage. You should be able to approve your smile before it is finalized. As you know, the final prosthetic component can cost you as much as a car! Make sure that you go somewhere you trust and can provide reliable follow-up over the long term. Ask them if they offer a guarantee on the implant-denture. I hope this helps!Ivan
