Revision - what should I do?

You did not post photos, but I suspect your sagging stretched tissue cannot support the extra weight and your own tissue has sagged. Your nipples are probably in the correct position, but the lower breast needs to be lifted. Reducing the lower breast would remove some excess weight and be less likely to sag again.
