Should a fascia covering be used for my rhinoplasty?
I'm booked in for a revision Rhinoplasty (many years ago had closed Rhinoplasty to reduce a large hook/bump). My consultant said they wouldn't be doing a fascia covering when they do my surgery. The plans are to deproject the nose, which I think will involve strut graft from my own septum cartilage, lift tip slightly, put spreader grafts in as my middle nose part is thin, and then will do an alar reduction as my nostrils will no doubt look a bit out of place/too long/flared when the job is done. I asked because I have thin skin, would they be taking skin to do a fascia/camouflage/cushion support, but they said no, there's not really a need, plus they said a fascia will build up the tip and they're working to reduce it so it would have opposite affect to what they're trying to do for my nose. I heard surgeons need to be very very precise when doing grafts without fascia covering as with thin skin through time can start to show through and look abnormal. I'm a bit worried, even though my surgeon seemed very confident. Last thing i want is to have the surgery and then a year later when my skin re drapes and shrinks that any bumps or cartilage ends up showing through my skin. Can anyone advise please?
