is it swelling that ever changes? you could try some antihistamiens and/or lymphatic massage ot see if htat helps (the lower eyelids are a common area for lymph trapping). the shape of it kind of looks like residual fat. Not sure if that means you need to get some more reduced vs the repositioning didn't quite work vs you might have had fat grafting too that hypertrophied. again, best to get an in person consult maybe wiht another surgeon since your surgeon is out of ideas.
