Are my alarplasty goals realistic? (Photo)

I've had 2 attempted alarplasties from the same doctor, I asked for him to remove enough that my nostrils line up with the inner canthus of my eyes (maybe 1mm or so more when smiling since when I smile my nostrils flare a lot) I thought me and my doctor had an understanding but after my second procedure my nostrils are still enourmous and bulbous and completely throw off my balance. I made it super clear to my surgeon what my goal was but it seems he keeps under-doing it pics are me now

Exercises cannot narrow the alar base. Sutures can be used but not very dependable. Alarplasty with excision of the skin is the only permanent and reliable way to reshape the alar base. Regards Dr. J
