Hoping to start HRT soon but wanted to know if my Gynecomastia boobs will become feminine and is there an option to do a fat transfer procedure? Notice my breast underarm fat and low nipple. Currently 42 B cup

In order to get more feminine breasts your will need some type of breast reconstruction. This would help to reshape your breasts, both skin envelope and fat/gland content and may require 2 or more  stages. It would be best to have in person consultation with a board certified plastic surgeon with experience in breast reconstruction. Good luck.
