Can submalar cheek implants create a more V shaped face? Mines is p oval. I have full lateral (or malar if thats what its called) cheeks but poor frontal projecting cheeks I think. I was also wondering if submalars can brighten my eyes, create a fuller shorter midface, and soften the prominence of my lateral cheekbones (by just a little bit). Also, should I consider a bit of fat grafting to temples and forehead to balance my face. Also should I use custom implants and what size should I use?

Yes, submalar implants will relatively narrow your face.However, v-line surgery too can help.We do both of those several times each week.  Check the videos to learn more.

Implants can be customized to the look you want. Symmetry and volume of the cheeks can be created with well-placed fillers (Voluma, Radiesse, Bellafill) and Sculptra. This will allow you to try on the look before using implants or fat. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer

While cheek augmentation can be beneficial in facial reshaping, the submalar implant style would be exactly what you wouldn't use. Such cheek implants add volume underneath the front part of the cheek making the lower cheek fuller. A V-shape require a high cheek (malar) augmentation combined with midfacial defatting. Such cheek implants are not a standard style and would have to be custom made to hug up against the lateral infraorbital and lateral orbital rims.

Submal implants will give you a fuller and the illusion of a shorter mid face, and soften the prominence of your lateral cheek bones. Fat grafting of the temples will also improve facial proportions from the front view. Implant size will be determined after physical examination; custom implant are rarely needed because there are many sizes and shapes available.
