Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. Fillers such as Restylane Silk, Belotero, or Juvederm can work well to treat lines around the mouth, but the best first option is laser resurfacing or deep peels to induce collagen production. Superficial radiofrequency devices and microneedling can help as well and are less invasive. I suggest going to an experienced cosmetic dermatologist with expertise in lasers and fillers. Thank you for sharing your photos and questions. From the photos provided it appears you may be a great candidate for skin resurfacing, neck and lower facelift, or both procedures. The lower facelift and necklift would address any gravity related facial aging, such as tightening the neck skin and jawline. Skin resurfacing (most frequently done with either a deep phenol chemical peel or with a CO2 laser) would take care of more of the fine lines and wrinkles. Patients seeking optimal improvement would usually opt for both treatments as they help improve facial aging in different ways (skin resurfacing for fine lines and wrinkles and necklift to improve sagging). In many cases, the skin resurfacing and necklift may be performed at the same time under general anesthesia or deep IV sedation. As I suggest to all my patients, it is important to prioritize your concerns and select procedures based upon your priorities. In your case, if the fine lines and wrinkles are top priority, and you aren't so much worried about the small amount of gravity related changes (jowling and loose neck skin), a single treatment of a fractionated CO2 laser under topical and local anesthesia might yield sufficient results to address your concern adequately. Thank you for sharing your question and posting your photograph. The erbium laser is excellent for improving fine lines and mild wrinkles. It will not get rid of your lines, but has very low risks. You should see significant improvements and might want a few treatments. Make an appointment with an experienced oculofacial surgeon who has an expertise with lasers. Thanks for the question. Your best treatment would be a facelift and fractional laser resurfacing. This could be done all at once.
Your best option is combination of laser resurfacing (preferably CO2) and periodic use of fillers. These together would help IMPROVE the wrinkles.
A combination of resurfacing preferably involving radiofrequency or laser, skin tightening that may be combined with PRP, microneedling and/or or chemical peels and fillers could give dramatic improvement. A good medical grade skin care regimen will also enhance and maintain results.
Responding to a 'call all PS'... I recommend medium or deep full face peeling, ie modified phenol peeling. Recovery over next 4-6 months then do surgical full face lift.
Great question! You would benefit from a combination treatment that would include hyperdilute radiesse on your lower face and neck, Juvéderm style hyaluronic acid fillers on the lower face around the lips, and CO2 laser series. Morpheus8 Microneedeling with RF also can add to improving skin texture and quality. This does not have to be done in one day, I usually recommend to do one or two treatments at a time, overtime.
