Answer: Recommend doing Morpheus8 to tighten up the skin Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. Answer: Vertical Lines / Smoker's Lines -- Fillers (Restylane Silk, Belotero), Laser Resurfacing, Peels, Superficial RF, Microneedling Fillers such as Restylane Silk, Belotero, or Juvederm can work well to treat lines around the mouth, but the best first option is laser resurfacing or deep peels to induce collagen production. Superficial radiofrequency devices and microneedling can help as well and are less invasive. I suggest going to an experienced cosmetic dermatologist with expertise in lasers and fillers. From the photos provided it appears you may be a great candidate for skin resurfacing, neck and lower facelift, or both procedures. The lower facelift and necklift would address any gravity related facial aging, such as tightening the neck skin and jawline. Skin resurfacing (most frequently done with either a deep phenol chemical peel or with a CO2 laser) would take care of more of the fine lines and wrinkles. Patients seeking optimal improvement would usually opt for both treatments as they help improve facial aging in different ways (skin resurfacing for fine lines and wrinkles and necklift to improve sagging). In many cases, the skin resurfacing and necklift may be performed at the same time under general anesthesia or deep IV sedation. As I suggest to all my patients, it is important to prioritize your concerns and select procedures based upon your priorities. In your case, if the fine lines and wrinkles are top priority, and you aren't so much worried about the small amount of gravity related changes (jowling and loose neck skin), a single treatment of a fractionated CO2 laser under topical and local anesthesia might yield sufficient results to address your concern adequately.
