Why are there indentations on each side of my nose? (Photo)

Question

I had primary rhinoplasty 2 months ago and my bridge was made more narrow. Osteotomes were performed and both upper and lower cartilages were trimmed. I now have dents on each side of the bridge just below the nasal bones. They are more apparent under a camera flash. Are these permanent? If so, how can they be fixed? Thank you.

Nasal sidewall indentations after osteotomies are likely the osteotomy site. This may improve or it may be corrected with a filler in this area. If the nasal shape and function are ok, time and/or a filler may be appropriate.

Indentations after rhinoplasty were likely associated with the dorsal hump takedown which you received. Irregularities at the cartilage nasal bone border can become apparent as early as 1 - 2 months after rhinoplasty. They will not likely improve and so a revision may be necessary. I would consult with a facial plastic surgeon to address your concerns.

It is hard to see exactly what you are talking about in the photo. Yes, the nose is narrower in the upper vault region.  Best to be seen in person, and of course you should discuss your concerns with your surgeon in person.

The healing process after rhinoplasty is that of contraction. If you already have indentations, as the swelling decreases this may not only get worse, but may have an effect on your breathing.You will probably need spreader grafts to re-support the upper lateral cartilages (where indentations are), for breathing and cosmetic improvement.

Of course it is still early and you need to wait longer. Your nose was greatly reduced in width. There appears to be a gap between the upper lateral cartilages and the nasal bones. You should wait to let the nose heal completely. Spreader grafts may be needed at some point if the gap bothers you. I am not a fan of injectable fillers in the nose. Talk to your primary surgeon about your concerns.

Unfortunately the photos are not adequate to see what is going on but based on your description you may have collapse of the upper lateral cartilages. I would bring this concern to your surgeonDr Corbin

Dear Og2250, Your photographs are to small to see what you are discussing however it sounds like the indentations you are concerned about may be from the osteotomy site.
