I have been performing medical microneedling (fractional microneedle resurfacing) with gratifying results for close to fifteen years using both manual dermal rollers (e.g. Dermaroller) and automatic devices (such as Dermapen) for treating all kinds of scars and for smoothing fine, superficial wrinkles and crinkles. The rational behind the modality is that the microneedles create microscopic wound channels within the skin that subsequently heal via new, native collagen and elastic fiber synthesis (neocollagenesis and neoelastogenesis), which results in improvement in tone and texture of scars, improves the appearance of dilated pores, and smoothes superfical wrinkles. Treatments tend to be relatively inexpensive and the downtime minimal--mostly a mild sunburn-like appearance for twelve to twenty-four hours afterward.More recently, certain companies have marketed radiofrequency microneedling devices that supposedly engender enhanced effects due to radiowave heating of the tissue via the needles, which is theoretically supposed to enhance the new collagen and elastic fiber synthesis. The supposed added benefits of these devices have yet to be fully substantiated by hard science. What can be said with certainty, however, is that the treatments are considerably more expensive and typically engender greater post-treatment downtime as compared with the less expensive, fewer bells and whistles, fractional microneedle resurfacing approaches mentioned above.
