I’m 48 yo, & average weight, & healthy My skin quality is still quite thick, I’m not showing wrinkles yet, though have had annual Botox for my 11 lines; though I’ve not had any surgical facial procedures. This said, I’m not opposed to doing so in the future as it’s warranted!! I have wide, fairly prominent cheekbones but also a fuller lower face regardless of weight fluctuations, & cheeks seem to be not only retaining their fullness but causing me to look more jowl-y than I’d like.

Your chin is a bit soft relative to your cheeks (which are great). If you look at the height from the base of your nose to the tip of your upper lip relative to the tip of your lower lip to chin, it is about 50:50. You want your lower lip to chin distance to be about 60% (1/3 upper lip, 2/3 lower lip/chin). By doing a chin implant or genioplasty, you could increase the height of your lower face and that would help create a better balance. A chin implant would help with your jowling and widen out the area just behind your chin. You could also consider doing a jawline liposculpt with genioplasty or chin implant. Either one would support your lower lip better as well.

Slimming the lower face is best achieved with buccal pad reduction and sometimes lower facial liposuction. It is difficult to determine if you are a candidate for these procedures from 2 photos. Go on some consultations and get a few opinions.
