Fractional CO2 laser and PRP results?

Thank you for your question. Skin resurfacing lasers are great, for fine lines, wrinkles, acne scarring, and overall texture of skin. However for deep marionette lines and naso labial folds, you need to replace lost volume in your upper and mid face. I like to use Voluma along with Sculptra to address these areas of deep volume loss and then work on the surface of your skin. I hope this helps you. Be well

Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Dark spots can improve with these treatments. I recommend fractional laser for incisional scars as well, especially after plastic surgery.

This is difficult to answer without pictures, but I do recommend a combination approach to get the best results for marionette lines and nasolabial folds. Fraxel/Clear + Brilliant are also great lasers for this. I recommend fillers for the folds and lines themselves but also lifting around the mouth and cheeks to build underlying anatomical support. In our practice we combine sculptra with other fillers "layered" on top with lasers, rf treatments, J Plasma and peels to get the most conservative natural look. Please see a facial sculpting expert. Our Fractional CO2 laser is often used to help decrease deep wrinkles, fine lines, and pigmented lesions. The resurfacing fractional technique encourages your body’s natural collagen cells to increase their production. When combined with PRP it works effectively to rejuvenate wrinkles, acne and scarring. I would not recommend using a RF fractional laser (We use the INMODE Fractora) in combination with fillers. Autologous fat transfer to the face is the most effective way to achieve a filler like effect without compromising the ability of the laser's techniques. Fillers such as sculptra or juvederm can be used, but if the patient is going under anesthesia, it makes more sense to use autologous fat because the supply isn't limited, it contains your own stem cells, and offers more of a chance for a semi-permanent result.

When combined with PRP it works effectively to with wrinkles, acne and scarring. For volume loss in the face such as marionette lines, I recommend using dermal fillers such as Voluma or Juvederm. Fractional lasers with or without PRP are fantastic for improvIng the surface characteristics of your skin. Wrinkles, brown spots, texture, etc. deep grooves and bulges are not corrected with fractional lasers. In some patients, fillers can be used to improve these areas of deeper volume loss or volume shift. For many patients, a facelift with fat grafting is the best solution. 

Fractional CO2 laser treatments are excellent for improving skin texture, fine lines, brown spots and decreasing pore size.  When combined with PRP, these results are even more dramatic and the healing time is decreased.  However, it is not a treatment that will significantly improve areas of volume loss such as the nasolabial folds or marionette lines.  These usually need to be treated with fillers, which add volume.  Sculptra is certainly an excellent choice as long as you are comfortable with multiple treatments probably being needed in order to get good results.  Another option is HA (hyaluronic acid) fillers, which show more immediate results that can be obtained in one visit.  I recommend a consultation with a physician who specializes in aesthetic medicine.

Fractional CO2 laser works the best for sun damaged, wrinkled skin. It seems to me that your problem is mostly due to lost volume, shift in fat fads and perhaps some bone loss.
In my practice I like to improve the nasolabial folds by replacing lost volume over the cheek bone area. Injecting the folds with a filler will just make the lower part of your face heavier and it is not esthetically pleasing. As for the marionette lines, fillers in the hands of an expert injector can make your lip area look younger and refreshed.  

Filler maybe the answer, depending on your expectations. Everything from HA fillers like Juvederm, through to Belotero and even Radiesse can help. There are new off label combination treatments like Ultherapy -Radiesse than can compound the positive effects of collagen stimulation and provide a realistic lift. All the best, Dr Davin S. Lim.
