Severe bruising and swelling during and after tear trough injection. I had tear trough injections yesterday, on my right side my lower bleph bruised and swelled immediately, it looked like a blood vessel was hit and literally blew up into a hematoma. My left swelled and bruised a little later. I've been icing all night without improvement. Is this related to ischemia into a vessel, or too shallowly placed?
Answer: Severe bruising and swelling after tear trough injection I am sorry to hear about your bruising. This can occur and is the most common risk of any injectable treatment. It does not appear that you have any ischemia or vascular occlusion from the filler. However, your bruising on the right side is severe. This will probably take a couple of weeks to completely subside. Icing is very helpful. You might also consider topical Arnica montana or Bromelain to speed resolution of the bruising. Answer: Tear Trough Fillers -- Try Cannulas; Icing, Vbeam Laser In the future use cannulas to limit these types of complications. For now, icing and pressure whould be helpful. Vbeam laser and venus legacy treatments can help speed the healing. Time and patience will help and it should clear up in a few weeks. Best, Dr. Emer.

I do not believe that you have ischemic issues, rather you have had bleeding into the tissues. Only your injector knows the depth of the injections. The tear trough is filled with small blood vessels and this is one area that I tend to use a blunt-tipped microcannula for injections. By using this technique, it is much less likely to get bruising, although it is still possible. In your case, you will probably have to wait out the swelling and bruising for some time.
