Planning to have 360 lipo + full upper back Lipo, upper arm Lipo, mini tummy tuck and fat transfer to inner calves. Hi, I’m interested in the above procedures, some of the surgeons are saying that it’s safe to perform all that in one go. I have abdominal muscle tear in my belly all the way up to my chest, which happened after the second pregnancy. That needs to be addressed. Surgeons say that it’s safe to remove 5-6 litres of fat from my upper body and inject 5 litres in my lower legs. I’m 40 y.o. Weight - 59 kgs Height - 158 cm 2 c-sections (5 years ago and 9 months ago) Please advise. Thank you!

Answer: Proposed procedures I’m not sure your proposed plan makes all that much sense. First of all it looks like you’ve had previous liposuction which is absolutely important to know if anyone is going to make an accurate assessment. It looks like your entire midsection has that liposuction and this is also apparently true for your arms as well. I could be wrong but that’s just how your body shows in your pictures. The correct treatment plan should be based on with the underlying primary, secondary and possibly tertiary diagnoses are. In your case I would say for your abdomen obesity with excess visceral fat and muscle separation aka diastasis recti are the biggest issues. Muscle separation from previous pregnancies is not a tear in the muscle but rather a thinning and stretching of the fascial layer and covering the abdominal muscles. A mini tummy tuck is not going to dress muscle separation and you don’t have much skin laxity. I don’t think you have 5 L of fat to remove because I think you’ve had previous liposuction. Also, fat transfer to your inner calves doesn’t seem indicated based on your pictures but I am not sure quite what you’re trying to achieve. Regardless I cannot imagine anyone grafting 5 L to your inner calves. My best recommendation for you is to have multiple in person consultations with highly experienced board-certified plastic surgeons. Find the most qualified a plastic surgeon and listen to their recommendations. This is how a proper surgical plan is put together. Patients putting together a plan of what they want on their wish list without proper consultation typically doesn’t work out as well as they had hoped for. In your case I think weight loss followed by a full tummy tuck is probably the most appropriate procedure if you want to have a flat abdomen. Liposuction of your back is a good source for fat and may give improvements in contour but this area does have skin laxity that contributes to shape and contour as well. To create a feminist S-curve fat can be grafted to the buttocks and outer thighs/hips. Find the right plastic surgeon, listen to their advice and typically patients get the best possible outcome. Patients have had a previous procedures should always include before and after pictures in order for us to make an accurate assessment. In the state of Florida (where I reside), there is a maximum of 1000 cc of liposuction allowed if liposuction is performed in conjunction with another procedure. 4000 cc is allowed if liposuction is performed as a stand alone procedure. This limitation is placed to prevent complications of blood loss and electrolyte imbalance associated with higher volumes of fat removal. 5000 cc lipoinjection to the calves seems somewhat excessive.
