Is it worth removing a small third nipple if the scar may look worse than it actually is?

It really depends upon the size and location of the nipple. Smaller nipples will leave a smaller scar. Nipples which are located along the breast fold leave a very well hidden scar. The final outcome will also depend on how you heal. Some patients tend to form keloids. Posting a picture would have greatly helped. 

It is difficult to say without a photo or examination. In most cases the scar will be less noticeable that the accessory nipple, particularly with attention to closure and scar treatment after the removal. If the nipple does not bother you, there is no medical need to remove it.
