Cirumcision scar and scrotum lightening - how can this be improved? (Photos)

I have quite a bit of discoloration on the shaft of my penis from the scar line to the head. I belive some of it is from finger placement during masturbation. Ive changed my habits but it doesnt seem to be going away. The skin also seems thicker and wrinkled in parts. My scrotum has a v on the bottom that seems to have gotten darker with age. Pretty self concious about these and would like to see if i have any options. Would prefer something i could do from home based on the area but open to other things as well.

First, I want to reassure you that variations in pigmentation along the circumcision scar line and scrotum are extremely common. Many men become concerned that these changes are abnormal, when in reality they are often the result of normal anatomy, friction, aging, and the way genital skin heals over time.

Regarding the discoloration on the penile shaft, it is possible that repeated friction over the years contributed to post-inflammatory hyperpigmentation, especially if there was chronic rubbing or irritation. However, once pigmentation has been present for a long time, simply changing habits may not cause it to fade significantly on its own. The thicker and more wrinkled texture you describe could also represent normal scar tissue remodeling from the circumcision itself, which can look and feel different from the surrounding skin.

As for the darker "V" pattern on the underside of the scrotum, increased pigmentation in this area is very common with age. The scrotum naturally contains more pigment-producing cells, and factors such as hormones, friction, genetics, and aging can all contribute to gradual darkening over time.

For home-based treatment, some patients see improvement with carefully selected pigment-correcting ingredients such as:

Azelaic acid
Niacinamide
Kojic acid
Tranexamic acid-containing products

However, the genital area is much more sensitive than other parts of the body, so treatments that work well on the face may cause irritation if used too aggressively.

If the primary concern is pigmentation, prescription lightening agents can sometimes help, although they must be used cautiously in these areas. If texture changes and scar thickening are significant, treatments such as laser therapy, microneedling, or scar-directed therapies may provide more noticeable improvement than topical products alone.

One important point is that complete removal of pigmentation differences is often unrealistic. The goal is usually improvement and blending of color rather than making the skin appear completely uniform. Fortunately, many patients are able to achieve meaningful cosmetic improvement with a combination of topical treatments and, when necessary, procedural therapies.

Based on your description, the discoloration sounds more consistent with benign pigmentation and scar-related changes than an underlying medical problem. The fact that you've already reduced friction is helpful, as preventing ongoing irritation is an important first step in stopping further darkening.
