Did I choose surgery years ago that is making me look worse now? (Photo)

32 years ago at age 32.5 I got a hard silicone chin implant when I had my rhinoplasty. I have been happy with the result until about 5 years ago when at age 59 I definitely noticed "jowls" or prejowl sulchus(?) I wear sunscreen on my face and neck and topical skin care but I feel like my face has dramatically sagged into a rectangular shape. Is this exacerbated by the type of implant or placement or is it just being 64.5 years old?

An in office examination and being able to palpate the implant will provide the most precise diagnosis.
