Deep tear trough line - Lower bleph? Filler? Tear trough implant? (Photos)

Hello experts! I have had 3 consultations with 3 different plastic surgeons who soecialize in face and eyes. All 3 of them gave me completely different recommendations, and all 3 of them advised AGAINST eachothers recommendations lol. So i a just so confused honestly and i am asking for a straight up answer. My ONLY concern, is my deep tear trough line. I do not mind my puffy eye bags, i actually think my eye bags are quite cute, and i dont mind when theyre a bit swollen. My ONLY concern is my tear trough line. My tear trough line is present 24/7 and doesn’t change with my eyebags. Heres are the options all 3 surgeons gave me. Surgeon 1: wants me to get a lower bleph, but instead of removing the fat bag, she would just reposition it lower to fill in the tear trough line. Surgeon 2: believes that surgeon #1 is wrong, and he doesnt like repositioning fat in that area because it can restrict blood flow and cause unnatural buldging. He believes that i dont need a lower bleph, and all i need is a fat transfer to my tear troughs to smooth the line out. Surgeon 3: believes that a fat transfer is not the right way to go, because fat transfers are still unpredictable, and alot of patients arent happy with their results, especially in such a thin skinned area. He said the fat transfer can be uneven and lumpy, and it can be out of the surgeons control. He suggested i get a comservative amount of filler in my tear troughs, using a cannula, OR a tear trough implant.

Three real options, each works in different anatomy.
Lower blepharoplasty with fat repositioning is my go-to when there's any fat herniation above the trough. It moves your own fat into the hollow, solves both issues with one operation, and lasts decades. No external scar. Best for patients in their 30s and older where some bag is starting to form alongside the trough.
Tear trough filler works when the trough is the only issue (no bag above) and you want a reversible option to start. The key is depth, filler has to be placed against the bone (periosteum) to look natural. Too superficial and it causes a bluish or puffy appearance. Skill of the injector matters more than which product is used. Restylane Eye Light, Belotero, and conservatively dosed Vollure are all common choices. Lasts 12 to 24 months typically.
Tear trough implant is the least common choice. It's a solid silicone or porous polyethylene implant placed against the orbital rim. Permanent, requires general anesthesia, and revisions are difficult. I rarely recommend it because the other two options handle 95 percent of cases better.
The decision usually comes down to physical exam findings. If when you push gently on your lower lid you can feel the fat pads bulge, you're a fat repositioning candidate. If the area is flat without herniation, you're a filler candidate first and revisit surgery only if filler doesn't satisfy.
For someone in their 30s or 40s with even mild fat herniation, surgical fat repositioning gives the most natural and durable result. For someone in their 20s with no fat issues and just a deep trough, filler is the right starting move.
Find someone who shows you the exact procedure on patients with starting anatomy similar to yours. The best surgeons can tell you in 60 seconds which category you fall into.

After looking at your photos and reading your concerns carefully, I actually think your instincts are important here: your main issue is not really excess puffiness or “bags,” but the deep hollow that creates the shadow under the eyes.
In your case, I would personally lean away from fillers as a long-term solution, especially because you are young, thin-skinned, and already have a naturally delicate lower eyelid area. While filler can sometimes soften a tear trough temporarily, this area is notorious for swelling, unevenness, and that slightly puffy or artificial look over time, even when injected well. Some patients love it initially but end up chasing maintenance or dissolving it later.
I also understand the hesitation about fat grafting. Fat transfer can work beautifully in the right patient, but under-eye skin is extremely thin, and fat survival can be somewhat unpredictable. Small irregularities that might not matter elsewhere on the face become very noticeable in the tear trough area. That does not mean fat grafting is “wrong,” but it does require a very experienced surgeon and very conservative technique.
Based on your photos, I actually think Surgeon #1’s approach makes the most anatomical sense if surgery is truly needed. You do appear to have a tear trough hollow that is related to the transition between the lower eyelid fat and the upper cheek. In those situations, carefully repositioning a small amount of your own existing fat can create a smoother contour because it addresses the actual structural change instead of simply “filling” on top of it. The key is conservative, precise surgery, not aggressive fat removal.
That said, I would also tell you not to rush into anything. Your under-eyes are not severe, and because you already like the natural character of your eyes, you want to preserve that. In someone like you, over-treatment is a bigger risk than under-treatment. If you decide to proceed, I would prioritize a surgeon who specializes specifically in lower eyelid surgery and consistently produces very natural results in younger patients.
You are asking exactly the right questions, and honestly, there is not one universally “correct” answer here, there are simply different philosophies. But if your goal is the most natural and durable improvement to the hollow itself, I would personally favor a conservative structural approach over repeated filler treatments. Hope that helps!

It can be frustrating to hear contradictory things from each doctor. The truth is, all three of those options are reasonable treatment plans for your concerns. What you are hearing are surgeons who have a preferred method because that is how they were trained/what they feel comfortable performing. All three options have the same goal of adding volume to the region but doing so in different ways. Filler in the undereye is risky due to the potential of malar edema, and generally I would recommend against unless you are unsure if you want a permanent solution at this point, as the rest of the options would be more permanent solutions. Fat grafting (surgeon 2) or fat repositioning (surgeon 1) are both great options with pros and cons to each. While it is difficult to predict how much fat will survive when grafting, in someone who is experienced in fat grafting the concern for lumps or irregularities should be minimal. In fat repositioning, this ensures the fat that is repositioned would survive (it keeps its blood supply as opposed to being a graft which has to grow a new blood supply), but it can end up being less total volume to the region when compared to fat grafting (a few cc's of fat when repositioning as opposed to maybe twice as much in fat grafting). I will say in repositioning, technically one of the fat "bags" would be moved, so you might lose that appearance to some degree (which you mentioned you like) with that approach.

I do not like implants and fillers are temporary.  The best technique is what was suggested by your 1st surgeon, transposition of the fat to fill out the tear trough.

I'm sorry that you have been inundated with conflicting advice and are going from pillar to post. The use of a low viscosity, malleable, HA filler in this case to elevate and smooth the tear trough would address several issues. It would smooth the transition between the upper cheeks  and the lower lid regions and would allow reversibility by dissolving--if the need should arise--something that would not be easily achievable if heavy, bulky fat was used, and it would avoid expensive, invasive surgery with all its potential complications,
I would further suggest that a real world test be performed before actually treating. This can be done simply by using just plain sterile saline solution or diluted anesthetic solution to ensure that the desired results could be obtained with the HA filler. 
The tear trough region is not a region to leave to the hands of novice injectors. You would be wise to consult with an experienced, board certified cosmetic dermatologist filler injector expert and best of luck.

Hi there, thanks for sharing your story and also your photos! I see that you were given a bunch of different recommendations, and it's definitely very hard for patients when being pulled in multiple different directions! 
I don't think any of those options are "incorrect". All of them will be able to address a deepened tear trough ligament, but it's the long term results that may differ. The gold standard here would be to perform a lower blepharoplasty with fat transposition (first surgeon). The procedure would break up that indentation and then the repositioned fat would help smooth out the transition from the eyelid to the cheek. However, this is a bigger procedure, much more invasive.
Fat transfer and filler can serve to "mask" this by filling in the indented area. This would smooth out the transition without larger incisions around the eyelid. Fat is more permanent and will not migrate. Filler in this location would be good for a temporary fix, but it has a tendency to migrate and there is a risk for blindness! Hope that helps
