Flat midface, recessed chin (Photo)

Question

I've been lurking around trying to find the best information so I can be knowledgable in my concerns and here they are -Exophthalmos due to weak orbital rim projection - flat midface -recessed chin I've considered genioplasty but I'm not sure what to do about the projected eyes and flat midface problem. I've heard everything from lefort 3 - zygomatic osteotony to fat transfers and everything in between.

Thank you for your pictures. Based on your pictures, I would recommend and sliding advancement genioplasty. Concerning your orbital rim area I would not recommend zygomatic osteotomy. It is technically challenging. Fat transfer may take multiple procedures  to achieve a modest improvement. I would recommend orbital rim/ malar implants. Please seek evaluation from a board-certified plastic surgeon with facial cosmetic surgery experience or a board-certified facial plastic surgeon

You can not move the bones of your face forward unless you are willing to adversely affect your bite. Thus a flat midface and infraorbital rims need to be augmented. Bone augmentation using custom designed implants would be the simplest and most effective method to bring the midface forward. They would create the best overall volumizing effect. The chin augmentation can be performed either by an implant or osteotomy based no what exact dimensional changes are desired.

When patients have a flat  cheek and maxilla profile, cheek implants  offer a permanent solution to give more projection to the cheeks. Cheek implants are composed of Silastic, are manufactured in a variety of sizes and shapes, and are inserted through intra-oral approach under a brief general anesthetic. Chin implants are also composed of Silastic, are inserted through a sub mental approach, when patients have a recessive and weak chin profile. Chin implants can be inserted under local anesthesia.
