I am wondering if any surgeon out there know how to create a philtrum dimple surgically. I know its possible to create philtral ridges with dermal grafts and combine it with removing subcutenous tissue in the area. I would still want it to be created without any foreign substances such as dermal grafts. Aint this possible ?

It is a bit difficult to fully evaluate the scar on your lip from the photo that you provided, but a scar revision can frequently be done to improve an uneven upper lip scar.
Lip reconstruction surgery is performed under local anesthesia in an office setting. A plan would be developed based on your goals for your outcome. It is very common to experience swelling after any lip procedure.
I believe to achieve the results you desire, you would need an excision of the tendon producing the two creases. I would perform a wedge excision, including the lower lip vermillion. I do not think it can be treated by a more conservative treatment.
