I am petite 5’2 100 lbs with 34 B breasts before and had 385 cc Silicone implants under the muscle with Periareolar incisions. My nipples are now pointing downward and very low on my breast. I'm really worried about how low of a position they are at now. My nipples were not low before the implants. They also feel and look too large for my small frame. They seem very abnormal compared to the other patients after implants. Do I need revision surgery or a breast lift? Can this be fixed?

(photo) Thank you for your question and photographs, and I am sorry that your current results have caused you concern. After placement of implants, the overall appearance of the breasts will change dramatically over the next several months, as the implants drop, and the overlying tissue stretches to accommodate the extra volume. Early in recovery the combination of tissue swelling and the volume of the implants causes a flattened, wide, and elevated appearance to the implants, while the lower breast appears empty and causes your nipples to point downwards. As this swelling resolves, and your tissues stretch, you should see gradual improvement. At six weeks I would anticipate these changes to start taking place shortly. I know waiting is difficult, but judge your results at 4-6 months after surgery as most of your breast changes should have occurred by then. Hope this helps

Answer: Get to your surgeona and find out why your nipples are as they are... as your pre-op photos suggest you should have had a great outcome with nipples pointing forward. Only your surgeon knows what was done and how you were closed but your photos suggest the closure technique is contributing to your funny look right now. In the meantime, focus on getting your implants where you want them and with upper poles being level so they look great in a tube top. If your mounds end up perfect but your nipples do not, a revision will make it better.
