I am female mid 30s and wondering which treatment is best for my tear troughs. They seem to look hollow and dark in some areas. The skin is also thin. Is filler suitable or PRP micro needling/or PRP injection a better option? And are these safe to do if I have visible blue veins under my eyes? I’ve also been told about threads and RF skin tightening and lasers for under eyes. Which of these options or any other treatments are best for me? and if lasers, which type do you recommend ? Thank you

Answer: An HA Filler Cheek Augmentation Not Only Creates A More Heart-Shaped Face, But Improves Under-Eyes A full-face frontal photo would have been very helpful here. However, there is a strong suggestion of volume deficit in the upper cheeks in the rightmost photo. If this is indeed so, then the use of a robust HA volumizing filler for the cheeks would not only help to create a more ideal, feminine, heart-shaped oval face, but would secondarily improve the under-eye areas, since volume loss in the upper cheeks is a major contributory factor to the development of infraorbital bags, sagging and crinkling. What's more adequate volume in that area shields any darkness that is contributed by the underlying blood vessels. Additionally, a series of superficial peels may further tighten the skin there and lighten the darkness. On the other hand, the use of expensive PRP treatments and energy-based gizmos, like fraxel lasers, etc., are far more backed by heavy marketing and advertising hype and social meda buzz than hard science. And threadlifting, which I gave up performing many years ago, likewise is expensive, has real post-treatment downtime, and poses a high risk for adverse reactions like the formation of divets, dents, infection and perforation of the threads. So, "buyer beware" for all of the foregoing. You would be wise to consult with an experienced, board certified cosmetic dermatologist. And my last word of advice, don't suggest treatments to the person you are consulting with because you unfortunately may very well get exactly what you ask for. and then be in for a whole lot of frustration and disappointment and out a whole lot of money. Answer: The best treatment for tear trough I have been saying this on Realself for what seems like forever. No hyaluronic acid fillers should go into the lower eyelid, ever. The single best treatment for tear trough is fat grafting. When a small amount of fat is removed from your own body where it’s not wanted, processed and using a cannula injected to the tear trough, the results are natural and permanent.
