I had a revision tip plasty almost 2 years ago and for the last six or so months my nose will get really cold at the tip and my thin skin shrinks tight around the nose and then you can see irregularities and the white cartilage through the skin at the tip. I have “paper thin skin” according to my surgeon and never used to before primary. lt’s driving me crazy because it happens when it’s just cool and feels so awful. I’m worried about winter again. Any suggestions? Accupunture? Why now? I can’t!

Definitely a rib can be removed via the closed approach no matter how it was secured. For simple removal or modification of a dorsal graft, there should be no reason to open the nose with a columellar incision.
