What options do I have to reduce the appearance of my scar? (Photo)

Hey, Hope all is well =) I have an about 5 inch scar on my shoulder that is seasoned (13 years old). Its raised and slightly hypertrophic. I had a shoulder plate and screws to fix a humerus fracture. I want to reduce the appearance of the scar. Any suggestions, options are appreciated !!! Warmly, Lynette
