Hello, and thank you for your question. I have utilized both the Sientra style 107 implant as well as the Inspira implant and I do believe they are both excellent choices. Most commonly we will choose a smooth round implant placed in the submuscular location depending upon the patient's choices and preferences. We have not had problems with rippling and wrinkling with this device. I would recommend that you seek out a qualified board certified plastic surgeon with experience in breast augmentation surgery for and in person consultation. Best of luck.
Hello and thank you for your question. You have a concern that may women share — whether their choice of implant will reduce or increase rippling and wrinkling. First of all, any implant will have some degree of rippling and wrinkling. This phenomenon occurs from the natural weight of the implant. Women can make different choices to avoid this, however.For example, smooth implants seem to have fewer instances of unwanted rippling and wrinkling than textured implants. Since textured shell implants better adhere to the breast pocket, their contents tend to shift more, causing rippling and wrinkling to occur. In addition, certain implants, such as the Sientra 107 and Natrelle® Inspira line, will have less occurence of rippling and wrinkling because they contain cohesive gels.Since cohesive gel implants retain their shape — even when cut in two outside the body! — they will tend not to ripple as much as implants which have more fluid contents, such as saline or non-cohesive silicone. (You may hear cohesive-gel implants referred to as “gummy” or “gummies” frequently.) Since the Sientra 107 and Natrelle® Inspira offer both smooth shell and cohesive gel options, you should definitely consider them if the possibility of rippling and wrinkling concern you. I disagree with my colleague who believes style 107 Sientra is teardrop. the 107 is a round highly cohesive implant. I prefer smooth round. The 107 is probably comparable to the Inspira SCM style . It sounds like you may have had Inspira SRM. The best approach is to consult with an experienced board-certified plastic surgeon who uses both Sientra and Natrelle. Below is an example of Sientra 106. Best wishes.
The 2 implants you mentioned are apples and oranges. The Sientra style 107 is a teardrop-shaped 5th Generation highly cohesive silicone implant. The Allergan Natrelle Inspira line are round silicone implants that come in 3 levels of increasing cohesivity. So, the first question is do you want round or teardrop shaped implants? The next question is whether you are going over or under the muscle. If you are going under the muscle, then I would not recommend the teardrop shaped implants. This is because the round ones under the muscle look about the same as the teardrop. Plus, the teardrop ones can rotate and may need a revision surgery to correct that rotation. If you are going over the muscle, then the Sientra 107 and the Natrelle Inspira level 3 cohesivity implant are probably about the same in terms of rippling. Be sure you have at least 2.5cm of upper breast thickness before going over the muscle. Your PS can measure this for you and help you decide.
As an early adopter of the gummy round devices such as the Sientra Style 107 and Natrelle Inspira Soft Touch and Soft Touch Cohesive, I feel I can truly provide a valuable reply to your question.Unfortunately there is a third type of Inspira implant that I haven't mentioned. That is the Inspira Responsive. Here's the low-down:The Inspira Responsive uses the softest and least cohesive of the silicone gel in these three devices. It was released into the US market October of 2015. It has the most rippling and upper pole folding in the upright position of these three Inspira implants. The implant was an improvement on older designs simply by increasing the implant fill from 85-95% of the shell capacity.The Inspira Soft touch uses a thicker more cohesive gel like that used in gummy bear implants. It was released into the US market in April of 2017. While providing improved smoothness and upper pole fill, I consider it to be the Goldlocks of implants: not too soft, not too firm.Lastly the Inspira Cohesive device has the thickest and most cohesive gel of any round device available in the US. This device was introduced into the US in September of 2016. I believe this device to be the pinnacle of smoothness (no other implant is likely to have less rippling without being too hard) and the most round and supportive of the upper pole. Some say that it can be too round and too firm, but when used in the right patient, it can do some pretty remarkable things.Now to the Sientra Style 107. In general, I would say that it has comparable upper pole fill and rippling to the Inspira Soft Touch. It may be slightly softer because of the technology Sientra uses to adhere the silicone gel to the implant shell requires less stiffness in the gel to achieve a similar level of stability and maintenance of upper pole fill. Otherwise, these two devices are very similar.My advice would be for you to seek out the opinion of a board certified plastic surgeon who has experience with these devices and discuss with him or her the best option for your particular situation.Best of luck.(This is an original reply authorized personally by Dr. Brian Brzowski. My experience is with the Sientra style 107 implant which is a round, smooth silicone implant. I have not had any rippling issues with this device, although it is possible with any breast implant. I have been using this implant for quite a while, and the patients who have had augmentation with it really like their results.
