Hi. I'm 5'2, 92 lbs. Approximately 6 wks ago I had gotten Textured Sientra Moderate Plus Implants, submuscular, 285cc. I really emphasized pre-op that I wanted to have cleavage but also a "natural look" that looks proportionate to my petite body frame. Should I have gotten a bigger implant perhaps with a high profile instead of moderate plus? If so, how big can I go without looking too unnatural but still being able to create cleavage. The gap is about 1 inch apart. Im not able to push my implants closer together currently.
Answer: Breast revision Dear TooPetite92, thank you for your photo and question. You are still in early stage of recovery. Your breast implants will require 4-6 months to settle. Only then you should be able to see the final result of your surgery. I would advise you to wait and see how will they look at the end. If you don't like the final appearance then you should visit a board-certified plastic surgeon for a consultation regarding the revision surgery. Answer: Are my implants too far apart? Regretting not going bigger At only 6 weeks postop, you are still early in your recovery with a great deal of swelling. There is often swelling over the sternum in the midline that pushes the implants laterally a little bit. As this swelling resolve, the implants tend to move slightly more medially, and the valley over the middle of the chest becomes deeper, resulting in greater cleavage. It typically takes 3 - 6 months for the swelling to resolve and the implants to settle into position after breast augmentation. I would recommend following up with your plastic surgeon to discuss your concerns. If you are still dissatisfied with your implant size and cleavage after 6 months, you could consider a breast implant revision. Best of luck. Ziyad Hammoudeh, MD

Answer: Cleavage Concerns I'm sorry to hear about your concerns. I don't think getting a higher profile implant would have helped you. At the same volume, a higher profile implant will increase its projection (how far the breasts stick out) but actually narrow the width. The key thing to keep in mind is that the implant should be centered on the nipple, otherwise a very unnatural appearance results. This can limit the amount of cleavage, especially in a more petite woman, like yourself. In my opinion, it is still too early to form any definitive opinions about your final result. More pictures would be helpful in getting a better sense of the implant pocket. I know it can be difficult to be patient, but see how the breasts settle and take it from there!

Answer: Breast asymmetry It is very difficult to provide you a definitive evaluation of your breasts from a single photo (it is not straight on view, one hand is raised and one isn't, etc.); From this single view it looks like your breast mounds are not symmetric; looks like the left is oblong and the right circular; Were you asymmetric to start? You may have needed more central dissection to allow the implants to be more centrally located; going with a high profile, only makes your breast mounds narrower and not the right answer for you. A consultation with a breast specialist and your preoperative photos are needed to give you a definitive answer.

Answer: Implants too far apart with breast augmentation, it is important to understand that your anatomy is the most critical factor in postop appearance. At 92 lb, it is unlikely that you could get a larger implant in the breast, depending on your specific breast diameter. The implants appear to be properly centered on the nipple, therefore it is likely that your breasts were somewhat laterally located preop and this appearance will remain postop. Sincerely, Thomas C.
