I am 55 years old. I am tall and slim, have been same weight (+/-5 pounds) all my adult life. My face looks OK, but developed sagging, wrinkled skin under my chin past 2 years. It's not fat, skin is very soft and sagging. I am getting married and want to look best in one year. What would give me best results in my case: Ultherapy, CoolSculpting® or Surgical Neck lift?

You have asked a great question and the answer is dependent on how much of an improvement do you want. Ultherapy is an options for you- you can review my before and after pictures for real patient results and you will see about a 20% improvement in the neck and jawline. The best results will be from a neck lift where the muscles are tightening under the chin and well placed incisions around the ear are used to remove the excess skin (you can see my neck lift pictures as well), and you will get an 80-90% improvement in the neck profile. The advantage of the ultherapy is that there is no downtime. Answer: Better resutls Neck lift would have a far better results than the other options, but you would also have larger expenses, longer recovery, and higher surgical risk. All are good options, but you should consult with a surgeon to see what your goals are and how to meet them. Be sure to discuss your goals and concerns with your board certified plastic surgeon and consider the pros and the cons. Be certain your surgeon is properly certified. A more invasive procedure called silhouette instalift will have approximately 40 to 50% improvement and a neck lift approximately 80 to 90% improvement. These procedures go from noninvasive to minimal and invasive spectrum. Of course an in person consultation and review of these options it would be best suited.

Better results may be obtained with a treatment like ThermiRF which may yield about a 40% improvement and is minimally invasive. Good Results may be obtained with Ulthera, which will yield about 20% improvement with zero downtime, though it typically takes 3-6 months to see results.
