What are my options for breakouts after Accutane?

Question

I completed an 8 month course of Accutane last week reaching a cumulative dosage of 170-175 mg/kg. Within the last few days, I began to break out again in areas that were previously clear. Is this common and what are my options to help reduce this new acne?

Accutane is a go-to medication for patients with recalcitrant cystic acne. It can reduce pore size and also reduce sebum production in your skin, making it less oily; and in turn, dryer. Accutane can be used to control acne, but patients receiving the medication should be carefully monitored. In terms of skincare, always wear an appropriate sunscreen and reduce direct sun exposure. In my patients, I recommend a good ceramide moisturizer such as Elevase whenever they are on Accutane. This will keep the skin hydrated and reduce the oiliness of the skin that is associated with oil-based moisturizers.

There are many options for post Accutane
treatment. Immediately after treatment a variety of washes, topical
prescription creams/gels, and even oral antibiotics can be prescribed. After
eight months to a year after Accutane treatment, retinoid creams and chemical
peels can also help.

There are many options after an accutane course to help maintain clear skin. I always suggest good medical skin care products that you can get from your dermatologists. 3 times per week, I prescribe topical retinoids (Vitamin A) for night time use for 1 year after accutane use. In addition, other topical prescriptions with BPO or clindamycin are available but topical vitamin A is preferred. Sometimes oral antibiotics and/or birth control are used and in severe cases, using accutane again is required. For the best results please consult a board certified
dermatologist with experience with your condition.
