There are a number of different terms applied to facial fat grafting. The procedure entails harvesting living fat cells from one part of the body and transferring those living cells to another part of the body. There are a variety of ways to process or prepare the fat. which may help improve the viability of the fat but there remains debate about which is the best way to harvest and prepare the fat. Nano fat designation refer to the size of the actual grafts being placed. It makes sense that the smaller the actually graft the more likely it is to survive but other factors, most notably the quality of the fat and the health of the host tissue bed also play important roles. No matter which technique is used some of the transferred fat will survive while some of the grafted material will die and be resorbed by your body. You never want to overcorrect with fat grafting as the result are long lasting and over correction is very difficult to fix. The target is a full correction but with some fat loss you can expect a mild to moderate under correction depending on fat survival. The grafts that do survive will last many years making the procedure very useful

Good question Bostongirl!Nano fat grafting involves using a device or a type of filter that when fat is pushed through it, debris is removed and smaller fat cells are collected. Some studies demonstrate better take of these smaller fat cells and the smaller cells appear to have a tendency to clump together less, providing a smoother filler quality. I do prefer this technique for fat transfer to the face and have enjoyed reasonable success compared to traditional fat grafting. It is basically the particle size that is used for grafting. Traditional structural fat has a larger, think of it more like an oatmeal kind of consistency, obviously it’s not that lumpy! Then as we take the fat through different layers Filters we can decrease the particle size. I usually call the next layer down micro fat and then as I get smaller I call it nano fat. Each of these is used for different purposes and at different levels. They are all very useful and any surgeon for sale with fat grafting will use all of these modalities. Best of luck! Dr. Boschert
