Thank you for sharing your case and photos.  I'm sorry that you have not had a good result after levator advancement and mullerectomy.  For patients who have recurrent ptosis after surgery, it is important to rule out a muscle weakness condition such as myasthenia gravis that may require medicine to treat rather than surgery.  Levator resection and levator advancement are basically the same operation.  After MG has been ruled out, the main thing that will determine which surgery is best for you is your amount of levator function.  If you have decreased levator function (usual for patients who are born with a droopy eyelid), you may benefit from a frontalis sling.  Consider a consultation with a board certified oculoplastic surgeon to discuss your concerns and to see which procedure would be best for you.  Good luck!

it does seem strange that your ptosis second repair didn't repair the eyelid height. there could be a number of issues here, and all would be answered with a good history and exam. was this a congenital ptosis (were you born like this)? was there trauma? in either case the levator muscle may not be working 'normally'. in that case, further surgery would depend on how much levator function you have and you may require a frontalis sling procedure to lift you lid to be even to the right side. if your answer to these questions is no, then I believe it is incumbent to rule out myasthenia gravis, which can cause recurrent ptosis. do you have any double vision as well?
