If there is an under eyelid issue, a blepharoplasty may be required. Please consult with a plastic surgeon for best results and assessment 

Hi there, It is difficult to figure out exactly what you mean without seeing photographs. I practice in Montclair, NJ and would probably use Restylane to fill the area under your eyes. I recommend you see a board certified dermatologist who has a specialty injecting under the eyes since this is a very tricky area. Good luck, Dr. Downie.
